Contact Chuck Zanone, Ph.D., LLC
Now offering remote telehealth sessions via a secure online video platform.
You may contact me using the form on the first page of my website. You may also contact me by telephone (706) 201-6921. I am often not immediately available. When I am unavailable, my telephone is answered by confidential voicemail. Please leave a message and I will get back to you as soon as I can. I will make every effort to return your call on the same day you make it, except for weekends and holidays. If you are difficult to reach, please let me know when you will be available.
Once you have started treatment with me, I am available for crisis, non-emergency calls. If you are unable to reach me and feel that you cannot wait for me to return your call, please contact your treating psychiatrist or family physician, or go to the nearest emergency room for assessment.
I do not use email to communicate with clients to protect their privacy. The only exception is use of encrypted email for initial contact and to provide initial paperwork and to send or receive consent forms.
If you are experiencing a life-threatening emergency, please call 911.
You will find links to crisis lines at the bottom of each page on this site.
If we agree to work together initial (intake) forms will be sent to you via encrypted email (Hushmail). All forms must be completed prior to starting treatment. Forms must be submitted at least two days prior to your first appointment. You may: return forms via encrypted email (Hushmail), fax them to me at (888) 858-1679, or leave them in my mailbox inside the front door of my office building.
Failure to submit paperwork in a timely manner will result in rescheduling your appointment.
